Please contact your Insurance Company for Prior Authorization as well as Deductable, Co-pay amounts. Insurance claims can be filed to your Insurance Company for you if all Insurance as well as personal information has been provided Or if you prefer, a claim form will be provided for self filing. Our office is not currently participating in network with any Insurance carriers, but we are working with Molina and workman’s compensation clients. Insurance claims will be filed with the status of out of network. All Counseling fees are due in full the day of service.
